Finish these online before your appointment so your provider can review your history in advance.

Every new patient completes a short set of registration forms before the first appointment. These let your provider review your medical history, current medications, and insurance details ahead of time.

Please finish the forms online before you arrive. Incomplete forms may mean the appointment has to be rescheduled.

What to Bring to Your First Visit

Five things that make check-in faster and your visit more productive.

A little preparation keeps your visit moving. Please bring:

  • A valid photo ID

  • Your insurance card, if you are using insurance

  • A list of your current medications and supplements, including doses

  • Any recent lab results or records from a previous provider

  • Questions you want to ask (it helps to write them down before the visit)

Your first appointment is about establishing care. Your provider will go through your health history, talk through any current concerns, and work with you on a plan that fits your goals.

Depending on what you need, this may include lab work, a prescription, a referral, or a follow-up visit. You will leave knowing the next step.

Starting Care by Telehealth

We serve patients in Alabama, Arizona, Colorado, Florida, Maryland, New York, Virginia, Washington, Washington, DC, Connecticut, and New Mexico.

You do not have to live near Hoover to become a patient. A virtual first visit works much like an in-office one. You complete the same registration forms, meet your provider by phone or video, and the team sends any lab orders to a location near you.

Insurance & Private Pay

Most major insurance plans are accepted. Affordable private pay rates available. Payment is due at the time of service.

Coverage varies by plan, so you are responsible for confirming your eligibility, copay, and any out-of-pocket cost before your visit.
